IAC12 maintains a practice waiver for the Sterling Municipal Airport (KSTK). See attached PDF.

Note that you don't need to activate the waiver unless you plan to perform aerobatics below 1500' AGL.  Sign the waiver at the FBO each time you open the waiver for use.
